When you come in for your appointment, please be sure to bring your medical insurance card and complete your Breeze registration using the invitation link sent to the email you provided.
Please bring a list of any medications and supplements you are taking as well as questions you may have regarding your health. Our providers are equipped to diagnose, document, treat injuries, and prescribe medication if necessary.
During your appointment, the physician will:

- Perform a basic physical exam to evaluate your overall health, although it may not be as thorough as an annual physical.

- Suggest a treatment plan, additional tests, or referrals to specialists if needed.
